Student Scholarship

- One $1500 will be awarded

- Qualified applicant must be:

  1. Pennsylvania resident actively enrolled in CAAHEP accredited perfusion training program or student actively enrolled in CAAHEP accredited perfusion training program located within Pennsylvania
  2. Involved in clinical rotation with overall GPA equivalent to “B” or greater

- Qualified applicant must submit the following application package to be considered:

  1. personal data form (see PSPS website)
  2. proof or residency if qualifying as Pennsylvania resident
  3. brief biography that will help us get to know you
      • Who are you?
      • How did you become interested in the perfusion field?
      • Previous education, work and clinical experience
  4. abstract of presentation (<250 words)
  5. letter from program director confirming active enrollment, active participation in clinical rotation and overall GPA equivalent “B” or greater
  6. Power Point Presentation
    • Relative to extracorporeal technology involving unique clinical scenario or innovative technique
    • Minimum: 12 slides / Maximum 18 slides

- Winning applicant is required to provide an oral presentation of Power Point submitted at PSPS meeting on Friday, April 13, 2012, in Pittsburgh, PA. Scholarship is awarded at conclusion of presentation. Travel and lodging will be the student’s expense.

  2. Fill out the form below and upload all required documents. You must have all required documents ready for upload at the time you complete the form. If not, you will be forced to start over as the session will not be saved
